Admissions
Our
admitting staff receives prior notice of the

admission, the patient or their designee must sign the Conditions
of Admissions Form to enable Kentfield to begin treatment.
Please have someone accompany the patient to the Hospital
if the patient is unable to sign for themselves. If a designated
person can not be present at the time of admission, please
make arrangements to complete the admission paperwork prior
to the patient’s arrival.
Kentfield also requires a copy of the insurance
cards, a copy of the patient’s advance directive/durable
power of attorney and additional information as requested.
The patient and family will be met by a member of the nursing
staff who will answer questions about the patient’s
care or accommodations.
Kentfield does not discriminate on the basis of race, color,
creed, religion, age, sex, national origin, handicap or
sexual orientation.

prior to the admission. The insurance company will be
notified promptly of the plan for a patient’s admission
in order to verify coverage, benefits, policy limitations
and to request authorization for payment of services.
For those patients who are not covered by a health plan,
financial arrangements will be discussed prior to the patient’s
admission.

Education Center is a dedicated space off the main lobby
provided for our patients and their families. The designed
area is equipped with a computer to access medical web sites
and stay in touch with loved ones. Also included are educational
pamphlets, videos or DVDs. The space also offers a quiet
room for a private meeting place for patients and their
families. Please speak with the receptionist regarding the
availability of the room.
Food Services
A
complete and nutritious diet is essential to a patient’s
recovery. The food service manager and dietician are
available to speak with anyone with questions, special

The Hospital food consists of fresh home cooked items with
attention given to a variety of dishes. The patient
is able to select food items from a daily menu. Kentfield

holidays are recognized.
Due to possible diet restrictions, the patient’s
