Overview
Kentfield
Hospital is a specialty hospital which provides expertise
to medically complex patients who may require extended acute
care hospitalization. Kentfield's physicians round on their
patients daily and the hospital provides qualified nursing
staff based on acuity and state guidelines for acute care
hospitals.
Kentfield Hospital
is licensed by the State of California Department of Health
Services as a general acute care hospital. The Center for
Medicare and Medicaid Services has certified Kentfield
as a long term acute care hospital (LTACH). LTACH facilities
bridge the gap between the critical care units and the
medical/surgical units of general acute care hospitals.
LTACHs are specifically designed to meet the needs
of patients facing extended hospital stays.
Kentfield Hospital provides complex medical treatment and
specialized rehabilitation needs in one setting. The
medical staff represents a broad array of specialists to
provide expertise to manage patients at risk for medical
crisis. Patients receive early access to interdisciplinary
services which prevents the future need to transfer to an
acute rehabilitation hospital.
Kentfield Hospital is not a skilled nursing facility or

Billing
The process for billing is initiated by the Admissions Department.
Benefits and eligibility is verified by the Hospital and
the health plans.
The
Admissions Department obtains authorization from the health
plan, which is required prior to transfer. Our case
management department obtains ongoing
authorization with the health plan’s Utilization Management
Department.
The Admissions Department
will check a Medicare patients full, coinsurance and lifetime
reserve days.
If Medicare does not cover 100% of the stay, a secondary
insurance is identified or patient is notified of their financial
responsibility for the co-pay portion of the bill.
Physician billing is separate from the hospital billing.
The Admissions office will facilitate a payment agreement
for physician services.
Referrals

Initial demographic information can be sent by fax to 415.485.3563.
Please note that for non-contracted health plans, Kentfield
can negotiate a one time Letter of Agreement.
Admission Criteria
Kentfield Hospital is a licensed acute care hospital specializing
in the care of medically complex and catastrophic rehabilitation
patients.
Patients considered for admission are hemodynamically
stable and have completed their initial diagnostic work-up.
Patients must have a diagnosis requiring extended acute
hospitalization.

Transfers
The Admissions Office and Clinical Liaison work with the
transferring facility to arrange for the patient transfer.
The health plan will always preauthorize transfers
prior to the admission.
Concurrent Review
While the patient is at Kentfield, the Kentfield Case
Managers will provide the health plan’s case managers
with ongoing reviews of the patient’s progress. Reviews
are given once a week unless otherwise requested.
Kentfield’s concurrent review process is focused
on insuring the patient receives the appropriate services
at the appropriate level of care.
