To make a referral to Kentfield Hospital, contact your clinical liaison or the Admissions Department at 415.485.3527. Initial demographic information can be sent by fax to 415.485.3563.

Please note that for non-contracted health plans, Kentfield can negotiate a one time Letter of Agreement.
