Referrals
Individuals may be referred for acute (inpatient) rehabilitation
if they can:
- tolerate three hours of therapy daily
- benefit from an interdisciplinary approach
- demonstrate practical improvements within a reasonable
period of time
- achieve realistic goals to allow a return to a less intense
level of care (such as a home, skilled nursing facility
or long-term care)
Individuals may be referred for long-term
acute care if they require daily physician visits and meet
one of the following criteria:
- need tracheostomy and/or ventilator weaning
- require I.V. antibiotics, parenteral nutrition or complex
wound care
- are unable to tolerate three hours of therapy daily
